ENTEK provides non-contributory life insurance to all employees.

Coverage amounts vary depending upon salaried and hourly status. Employees have the opportunity to elect additional supplemental life insurance coverage. The cost to employees for contributory life insurance is based on the age of an employee and the amount of insurance desired.

Employees can elect to insure their spouse for up to one-half of the employee's insured amount. The cost for spouse insurance is based on the spouse's age and the amount of insurance desired.

A non-contributory dependent insurance of $1000 is provided by the Company.
